Health Club Reimbursement Program

Health Club Reimbursement Form

Caremark cares about the health of its employees, which is why we provide employees with an annual reimbursement of health club membership fees, up to $250, for use at any gym or fitness center.



Health Club Reimbursement Eligibility

All employees working outside of the Northbrook, Scottsdale, and Tri-State facilities, and who work at least 20 hours per week, are eligible for the $250 reimbursement. Northbrook, Scottsdale, and Tri-State employees are not eligible because they have daily access to a free, on-site facility.

This reimbursement is for employees only; other family members are not eligible.



How it Works

Once you have incurred and paid your health club membership fees, you may apply for reimbursement by submitting a Health Club Reimbursement form and your health club membership receipts to the Caremark Benefits Center. Health Club Reimbursement forms may be obtained by clicking here, or by contacting the Caremark Benefits Center. Please note that:

Eligible expenses must not have been incurred before your hire date.
Reimbursement is provided once per rolling calendar year (every 12 months). For instance, if you receive reimbursement in December of 2007, you will not be able to receive reimbursement again until December of 2008.

Once your Health Club Reimbursement form has been approved by Benefits, you will receive your reimbursement in your payroll check. Current IRS regulations consider fitness reimbursement programs as taxable income, and therefore, reimbursements will have taxes withheld. To keep your benefit whole, however, your reimbursement will be “grossed up” so that your net reimbursement is the price of your membership, up to $250.
