Health Risk Assessment (HRA)

Caremark is offering you the opportunity to participate in the University of Michigan Health Risk Assessment (HRA). Learn more about your own health and your own health risks by taking this 20-minute confidential health survey. After you finish the health survey, you will receive a personalized health report that will offer you strategies and tips on how to improve and/or maintain your own good health. Studies show that by achieving better health and reducing your risk factors, you can help decrease the amount of money that both you and the Company spend on healthcare.

Click here to access the HRA – You will need to login to Employee Self Service, select Benefits, and click on the link to the health Risk Assessment.
