What happens if I cancel my participation?
You may cancel your participation by suspending contributions from CareStock at any time by calling AST’s 24-hour, toll-free phone number, (866) 860-0367. As long as you request a suspension on or before the 20th of any given month, your participation will end on the first pay period of the following month. Keep in mind you must re-enroll if you choose to participate in CareStock in the future. What happens if I leave work temporarily?
If you leave work at Caremark Rx due to a leave of absence, you may continue to participate in CareStock, as long as you continue to make contributions through payroll deductions. If you are on an unpaid leave, you will not be able to contribute. However, stock will be purchased on your behalf at the end of the stock purchase period with any contributions deducted from your paycheck during that period. What happens when my employment ends?
Your participation in CareStock will end if your employment with Caremark Rx, Inc. ends for any reason including your: • Termination of employment |
| •Retirement |
| •Disability |
| •Death |
What happens if CareStock is amended or terminated?
Caremark Rx, Inc. reserves the right to amend or terminate CareStock at any time. Upon any amendment to the Plan, shareholder approval will be obtained if required by law. However, CareStock will continue until the earliest of the following occurs: | •The Board of Directors terminates CareStock |
| •No shares remain to be purchased under CareStock |
If CareStock is terminated, unless otherwise specified, stock will be purchased in your name on the termination date. Any accumulated payroll contributions not used to purchase stock will be refunded through payroll as soon as administratively possible.
